Community Class Liability Waiver

This agreement releases Rocky Mountain Rhythm from all liability relating to injuries that may occur as a willing participant in dance-related activities. By signing this agreement, I agree to hold Rocky Mountain Rhythm and Motion Dance Center entirely free from any liability, including financial responsibility for injuries incurred. 

I fully acknowledge the risks involved in dancing and agree that I am participating voluntarily, signing that I have been made aware of the risks involved. I do not have any conditions that would increase my likelihood of experiencing injuries while engaging in this activity.

COVID-19 Release:

I agree that I am personally responsible for my safety and actions while attending in person classes at Motion Dance Center facilitated by Rocky Mountain Rhythm.  I agree to comply with all policies and rules, including but not limited to guidelines, signage, and instructions. With full awareness and appreciation of the risks involved, I hereby release, waive, discharge and covenant not to sue Motion Dance Center, Rocky Mountain Rhythm, its affiliates, employees, and independent contractors from any and all liability and claims whatsoever directly or indirectly arising out of or related to any loss, damage, or injury, including death, that may be sustained by me related to COVID-19.

By attending class, I forfeit all right to bring a suit against Rocky Mountain Rhythm, Motion Dance Center, and its affiliates for any reason at any time. I will make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed.
